  • An EX-24Model is proposed to address causation at interface of railway–related organizations.
  • A high-profile train accident in Taiwan is used as a case study to demonstrate the applicability of the EX-24model.
  • Key safety issues regarding interactions between the railway company and external parties are identified.
  • Research adds to existing knowledge in practical railway safety management and causation modeling.
